This form is for the registration of the State Board of North Carolina, Cosmetic Art Dept., approved continuing education courses: "The Arts, Science and Technology of Hair Designing - PART I, & II,

One Pre-Registration form per attendant

Term & Conditions:On-line Registration may be accepted up to 5 business days prior to date of class. To ensure admission, submit your payment by mail at least 5 days prior to class date. Late On-line Registration maybe accepted, if space is available, request permission via email no later than the Friday before classes begin. If you make it to class before you registration and payment is received, your hours will not be reported until payment has cleared our bank. This is risky because you may end-up paying an extra $75.00 to have your hours reported separately from your class-mates, per State Board's rules.

Cancellation notice must be received, via email , redeialb@yahoo.com , at least 5 business days before the class begins in order to receive a refund. Your payment will be returned to the address on the registration form, within 3 business days of your email cancellation notice. No refund will be issued after cancellation period stated above.

Please print this form and mail it with your payment, keep a copy for your record; however, it will not serve as proof of registration or payment. Once this form along with payment is accepted, you will receive an expressed email receipt and confirmation of your course reservation, by U.S. mail if no email address is listed below.
